Permethrin, the active ingredient in the cream, is a pyrethroid, which serves as a scabicidal agent used to treat Sarcoptes scabiei (scabies) infestations. It works by interfering with the nerve cell membranes of the parasites, specifically by disrupting sodium channel currents. This causes delayed repolarization, leading to paralysis and eventual death of the parasites.
Permethrin is primarily administered topically, with minimal systemic absorption. Studies show that less than 0.5% of the applied amount is absorbed within the first 48 hours. Once absorbed, it undergoes rapid metabolism through ester hydrolysis, mainly in the liver. The resulting metabolites are primarily excreted in the urine.
